- Your insurance card
- The practice demographic forms we emailed or you printed from this site
- A list of any medications or supplements you currently take
- Any other pertinent information we may have requested at your initial phone discussion
Cancellation Policy
We typically reserve an hour of time for patient visits. Please notify us at least 24 hours in advance if you need to reschedule or cancel your appointment so that we may offer that appointment slot to another patient. Thank you!
Payment Methods
We accept Cash, Visa, MasterCard, Discover and American Express as methods of payment. All insurance co-pays are due and collected at the time of your appointment. If you should have any questions relative to insurance and payment policies, we are happy to assist.
